It works for me. I resent the raw MIME message through the gateway with PDF encryption (OTP mode) and the encrypted PDF contained the attached PDF.
Are you sure that this is not just a PDF reader issue? Some PDF readers do not show any attachments (Adobe Acrobat does)

I had similar issues with Adobe. Can you see if it works using foxit reader? Can you also send me an encrypted message with attachment?
